Recent Texas crashes and commercial-vehicle inspection results show what neglected tire maintenance can cost.

On Aug. 14, 2026, a westbound tractor-trailer suffered a tire blowout on Interstate 20 in Martin County. According to preliminary information from the Texas Department of Public Safety, the truck crossed the center median and entered the eastbound lanes, where it collided with two other tractor-trailers. One man died, and two drivers were taken to a hospital with minor injuries. The investigation remains open.

A blowout can feel instantaneous from the driver’s seat, but the tire may have been deteriorating for hundreds of miles. Low pressure, excessive heat, worn tread, cuts, leaks, impact damage and improper loading can weaken a commercial tire long before its casing ruptures or its tread separates.

Truck tire blowouts are too often treated as random highway emergencies. The evidence points to a harder truth: unsafe tire conditions remain common, and consistent pressure checks, inspections and repairs can prevent many failures before a truck enters traffic.

Unsafe commercial-truck tires are not rare inspection findings

During the Commercial Vehicle Safety Alliance’s three-day 2025 International Roadcheck, inspectors conducted 44,435 vehicle inspections in the United States, Canada and Mexico. They found 2,899 tire-related out-of-service violations. Tires were the second-most cited vehicle problem and accounted for 21.4% of all vehicle out-of-service violations.

These were not cosmetic problems. The cited conditions included flat tires, insufficient tread, severe cuts exposing internal cords, bulges, improper repairs and tires not rated for the load or for highway use. An out-of-service condition is serious enough that it has to be corrected before the vehicle can continue operating.

Crash data show the consequences. The National Highway Traffic Safety Administration reports that 511 people died in tire-related crashes in 2024. That figure includes all vehicle types, but it establishes that tire failure is not a minor roadside inconvenience. In large-truck fatal-crash data for 2022, the Federal Motor Carrier Safety Administration listed tires among the most frequent offenders.

Inspection results and crash records measure different things, but they support the same conclusion: despite the industry knowing the importance of tire maintenance, tires in dangerous conditions continue to reach public roads. A carrier that waits for a driver to feel vibration or hear a tire fail has waited too long.

What happens when a truck tire blows out?

A blowout is a rapid loss of pressure caused by a rupture in the tire. The casing may split, or the tread and steel belts may separate before the remaining structure gives way. The force can tear away tread, damage the wheel well and nearby equipment, or throw heavy rubber into surrounding traffic.

What happens next depends on the tire’s position. A steering tire blowout can pull the truck toward the failed side and immediately make steering difficult. Failures on a drive or trailer axle can destabilize the truck, damage other components or cause the trailer to move unpredictably. At highway speeds, even a skilled driver may only have seconds to keep the truck in its lane.

Why maintenance is central to truck tire blowouts

Underinflation creates destructive heat

A tire’s sidewall flexes every time it meets the road. When the tire is underinflated, it flexes more than designed. That extra movement creates heat and can weaken the casing, belts and bond between internal layers. The tire may continue rolling without an obvious exterior warning until the accumulated damage results in tread separation or a rupture.

A pressure gauge provides information that a visual check or a strike with a tire thumper cannot. Dual tires can make a low tire even harder to recognize because the neighboring tire may carry more of the load while the damaged tire continues to flex and heat up.

Impact damage can remain hidden

A pothole, curb strike or roadway object can damage a tire’s belts or inner liner without producing an obvious cut on the outside. Continuing to run that tire, particularly at low pressure, can turn a repairable condition into a blowout.

Cuts, leaks and worn tread are service warnings

Federal tire rules prohibit operating a commercial vehicle with several visible hazards, including exposed belt or ply material, a flat tire or an audible leak. Under 49 C.F.R. Section 393.75, front tires on a truck or truck tractor need at least 4/32 inch of tread in a major groove; most other tire positions need at least 2/32 inch.

Those are minimum legal thresholds, not a substitute for a maintenance program. Repeated air loss, uneven wear, bulges, exposed cords, questionable repairs and recurring alignment problems all call for attention before the tire is returned to service.

Overloading concentrates stress

Each tire has a load rating. Excessive weight or uneven distribution can place more stress on a particular tire or axle position than the operator realizes. Underinflation makes the problem worse because the reduced air pressure leaves the tire less able to support its assigned load.

Texas heat accelerates an existing problem

Hot pavement and highway speed increase the heat and mechanical stress placed on a tire. Heat can accelerate damage caused by low pressure, overloading or internal deterioration, but it is rarely a complete explanation by itself. Blaming the weather can obscure the maintenance condition that allowed the tire to overheat.

The NTSB documented how missed maintenance can become a fatal blowout

A National Transportation Safety Board investigation into a 2023 motorcoach crash in Wawayanda, New York, shows why a blowout should not be dismissed as bad luck. A left-front steer tire failed as the bus traveled on Interstate 84. The bus left the road and rolled down an embankment. Two adult chaperones died, 15 people suffered serious injuries and 27 had minor injuries.

The NTSB found that the tire had operated underinflated for an extended period and had earlier internal impact damage. Its tread and belts detached, and the casing ruptured. The agency also found that the carrier’s inadequate inspection process allowed the bus to operate with underinflated tires.

A maintenance inspection two days before the crash marked the tires ‘OK,’ yet the form had no field for recording a manual pressure reading. That finding exposes a serious problem: A tire can look acceptable and still be running at a damaging pressure.

Recent Texas blowouts show how quickly drivers can lose control

Martin County: one blowout led to a three-truck crash

DPS’s preliminary account of the Aug. 14, 2026, I-20 crash says the westbound tractor-trailer suffered a blowout, crossed the median and collided with two eastbound tractor-trailers. San Antonio: a blowout brought down an overhead sign

On March 12, 2026, an 18-wheeler lost control after a tire popped on southbound Interstate 35 near Somerset Road, according to the San Antonio Police Department. The truck entered a grassy median and struck an overhead sign, which fell across the highway. No major injuries were reported, but the crash closed the roadway for hours.

Neither preliminary report proves that maintenance was neglected. Both incidents show why the answer cannot stop at ‘the tire blew.’ Investigators need to determine what failed, how long the damaging condition existed and whether a proper inspection would have found it.

Federal rules place tire safety duties on drivers and carriers

Under 49 C.F.R. Section 392.7, a commercial driver may not operate a vehicle until satisfied that its tires, wheels and rims are in good working order.

The motor carrier has a separate obligation under 49 C.F.R. Section 396.3 to systematically inspect, repair and maintain vehicles under its control and keep parts and accessories in safe operating condition.

These rules reflect the reality that tire safety cannot depend on a single glance before a trip. Drivers need a meaningful pre-trip check, and carriers need a maintenance system that tracks pressure loss, recurring defects, repairs, tire age and replacement decisions.

A blowout does not automatically prove violation or negligence. Concealed impact damage can cause a sudden failure even when reasonable care was used. But when records show low-pressure warnings, repeated leaks, worn tread, deferred replacement or inspections that existed only on paper, the blowout may have been preventable.

Evidence that can show whether the blowout was preventable

The failed tire is often the most important piece of evidence. Its condition can help a qualified examiner distinguish prolonged underinflation from a puncture, impact damage, an improper repair or another failure mechanism. Tread pieces and the valve stem may also contain evidence that is lost if the truck is repaired and the debris is discarded.

A focused tire-blowout investigation may include:

  • The failed tire, tread pieces, wheel, valve stem and related components.
  • Manual pressure readings, monitoring-system alerts and automatic-inflation data, when available.
  • Pre-trip inspection reports, driver defect reports and repair certifications.
  • Tire purchase, rotation, repair and replacement records.
  • Alignment records and reports of uneven or recurring tread wear.
  • Load and axle-weight information relevant to the failed tire’s position.
  • Photographs and video showing the tire, debris field and truck immediately after the crash.

A generic entry like ‘tire blowout’ in a crash report describes the event, not its cause. Physical evidence and maintenance records are what really reveal whether the tire failed without warning or after warning signs went unaddressed.

  1. Call 911 and move away from active traffic if you can do so safely.
